My Aged Care and Exercise Physiology
Your aged care funding can cover Exercise Physiology at Beachside EP.
My Aged Care is the Australian Government's entry point for government-funded aged care services. Through My Aged Care, eligible older Australians can access funding that covers allied health services β including Exercise Physiology, Dietetics, and Diabetes Education β from Beachside Exercise Physiology.
There are currently two main programs relevant to allied health: the Support at Home program (which replaced Home Care Packages from November 2025), and the Commonwealth Home Support Programme (CHSP), which continues until at least July 2027. Both can fund our services.
We accept referrals from care coordinators and case managers and provide detailed written reports back to your GP and care team after every review appointment.

Commonwealth Home Support Programme
CHSP is designed for older people who are mostly independent but need some help to continue living safely at home. It is the entry point for many people new to aged care.
- For Australians aged 65+ (50+ for First Nations people)
- Entry-level services β not a packaged budget
- Covers individual allied health sessions including Exercise Physiology
- Continues until at least July 2027
- A small client contribution is usually required
Replaced Home Care Packages from 1 November 2025
Support at Home provides more intensive, coordinated support for older Australians with more complex care needs. It replaced the former Home Care Packages Program.
- 8 funding classifications (up from 4 HCP levels)
- Annual funding up to $78,106 depending on classification
- Allied health funded under Clinical Care and Restorative Care pathways
- Former HCP clients transitioned automatically in November 2025
- Means-tested contributions apply

Our allied health services are well matched to what aged care programs fund under Clinical Care and Independence Services categories. All three services are available in-clinic in Mordialloc and via telehealth.
Exercise Physiology β Strength, Mobility & Chronic Disease
Our Accredited Exercise Physiologists (AEPs) design and supervise safe, evidence-based exercise programs for older adults. Whether you are managing a chronic condition, recovering from a fall or surgery, or wanting to maintain your independence and strength as you age β we build a program around you and your goals.
Dietetics β Nutrition for Healthy Ageing
Our Accredited Practising Dietitian works with older adults to address nutrition needs that change with age β including unintended weight loss, malnutrition risk, diabetes dietary management, digestive issues, and building a diet that supports muscle maintenance and energy levels.
Diabetes Education β Managing Diabetes Well in Later Life
Our Credentialled Diabetes Educators (CDEs) provide structured education and support for older Australians living with Type 1 or Type 2 diabetes. We help with glucose monitoring, medication understanding, lifestyle strategies, and working alongside your GP and care team for coordinated management.

Getting started is straightforward.
Whether you are new to aged care funding or already have an existing package, here is how to access Beachside EP as your allied health provider.
Check your eligibility through My Aged Care
If you have not yet accessed aged care funding, start by calling My Aged Care on 1800 200 422 or visiting myagedcare.gov.au. They will guide you through an assessment to determine which program you are eligible for. You need to be assessed before accessing government-subsidised services.
Talk to your care coordinator or case manager
If you already have a Support at Home package or CHSP services, contact your care coordinator. Let them know you would like to add Exercise Physiology, Dietetics, or Diabetes Education to your care plan. They will arrange a referral to Beachside EP. If you are self-managing your package, you can contact us directly.
Contact Beachside EP to arrange your appointment
Once you have a referral β or are self-managing and want to proceed directly β call us on 0409 051 260 or email admin@beachsideep.com.au. We will confirm your funding type, discuss which clinician best suits your needs, and book your first appointment. In-clinic and telehealth options are available.
Your first appointment β a thorough assessment
Your first session is an extended consultation where we take a full health history, assess your current function, strength, or nutritional status, and set goals with you. We write a detailed report back to your GP and care coordinator. Family members or carers are always welcome to attend.
Ongoing care, reviewed regularly
We provide structured, ongoing programs with regular progress reviews. Reports are sent back to your GP and care team after every review, keeping everyone informed and your care plan current. Sessions are available in-clinic or via telehealth.

Referring your clients to Beachside EP.
We make the referral process easy for care coordinators and case managers. We accept referrals by phone or email and provide detailed written reports back to you after every review appointment.
Our clinicians offer Accredited Exercise Physiology, Dietetics, and Credentialled Diabetes Education β all from a single allied health practice in Mordialloc. We see clients in-clinic Monday to Saturday and via telehealth for those who prefer or require it.
We are experienced working within aged care systems and understand the reporting and coordination requirements of both CHSP and Support at Home.
