Aged care funding for allied health that works

If you're an older Australian accessing government-funded aged care — or a family member helping someone navigate it — Beachside Exercise Physiology can help you use your funding for real, clinically-led exercise, nutrition and diabetes care.

Important update: As of 1 November 2025, the Home Care Packages Program has been replaced by the new Support at Home program. The CHSP continues until at least July 2027. Existing clients transitioned automatically — no action was needed.

Accessing Beachside EP through My Aged Care

My Aged Care is the Australian Government's entry point for government-funded aged care services. Through My Aged Care, eligible older Australians can access funding that covers allied health services — including Exercise Physiology, Dietetics, and Diabetes Education — from Beachside Exercise Physiology.

There are currently two main government programs relevant to allied health: the Support at Home program (which replaced Home Care Packages from November 2025), and the Commonwealth Home Support Programme (CHSP), which continues until at least July 2027. Both can fund our services.

Which program applies to you?

Commonwealth Home Support Programme (CHSP)

Entry-level support for mostly independent older Australians

CHSP is designed for older people who are mostly independent but need some help to continue living safely at home. It's the starting point for many people new to aged care.

For Australians aged 65+ (50+ for First Nations people)

Replaced Home Care Packages from 1 November 2025

Support at Home provides more intensive, coordinated support for older Australians with more complex care needs. It replaced the former Home Care Packages Program.

Diabetes Education — Managing Diabetes Well in Later Life

Our Credentialled Diabetes Educators (CDEs) provide structured education and support for older Australians living with Type 1 or Type 2 diabetes. We help with glucose monitoring, medication understanding, lifestyle strategies, and working alongside your GP and care team for coordinated management.

Getting started is straightforward

Whether you're new to aged care funding or already have an existing package, here's how to access Beachside EP as your allied health provider.

Check your eligibility through My Aged Care

If you haven't yet accessed aged care funding, start by calling My Aged Care on 1800 200 422 or visiting myagedcare.gov.au. They'll guide you through an assessment to determine which program you're eligible for. You need to be assessed before you can access government-subsidised services.

Talk to your care coordinator or case manager

If you already have a Support at Home package or CHSP services, contact your care coordinator or case manager. Let them know you'd like to add Exercise Physiology, Dietetics, or Diabetes Education to your care plan. They will arrange for a referral to Beachside EP. You can also self-refer if you are self-managing your package.

Contact Beachside EP to arrange your appointment

Once you have a referral (or are self-managing and want to proceed directly), call or email us. We'll confirm your funding type, discuss which clinician is best suited to your needs, and book your first appointment. We offer both in-clinic appointments in Mordialloc and telehealth options.

Your first appointment — a thorough assessment

Your first session is an extended consultation where we take a full health history, assess your current function, strength, or nutritional status, and set goals with you. We write a detailed report back to your GP and care coordinator after every review. Family members or carers are always welcome to attend.

Ongoing care, reviewed regularly

We provide structured, ongoing programs with regular progress reviews. Reports are sent back to your GP and care team, keeping everyone informed and your care plan current. Sessions are available in-clinic or via telehealth.

Aged care funding FAQs

Do I need a referral to access Beachside EP through aged care funding?

For Support at Home and CHSP, a referral from your care coordinator or care manager is the usual pathway. If you are self-managing your Support at Home package, you can arrange services directly. For CHSP clients, referrals are typically coordinated through your assigned Regional Assessment Service or care coordinator. Call us and we'll help you figure out the right process for your situation.

What happened to Home Care Packages?

Home Care Packages (HCP) were replaced by the new Support at Home (SAH) program on 1 November 2025, as part of the Australian Government's aged care reforms following the Royal Commission into Aged Care. If you had a Home Care Package, you were automatically transitioned to Support at Home — you didn't need to reapply, and your services continued without interruption. The new program has 8 funding classifications (compared to 4 HCP levels), with maximum annual funding of up to $78,106.

How much will my sessions cost?

Under CHSP, a small client contribution is usually required — the amount varies based on your income and services received, and is agreed upon before services begin. Under Support at Home, costs depend on your funding classification and means-tested contributions. In many cases, your allied health sessions will be fully or largely covered by your package. Contact us and we'll discuss costs transparently before your first appointment.

Can I access services if I'm still on the CHSP?

Yes, absolutely. The CHSP continues until at least 1 July 2027. If you are currently receiving CHSP services, you can still access Exercise Physiology, Dietetics, or Diabetes Education through your CHSP funding, provided allied health has been included in your approved services. Check with your care coordinator.

Is telehealth available for aged care clients?

Yes. Many of our older clients prefer telehealth for review appointments or when travel is difficult. We offer video consultations for Exercise Physiology, Dietetics and Diabetes Education. Family members can join via video too, which many find helpful. Telehealth sessions are available under aged care funding pathways.

Can my family member or carer attend appointments with me?

Absolutely — we encourage it. Family members and carers are welcome at any appointment, either in-person or via video. Having a familiar person present often makes the process more comfortable and helps ensure the care plan is followed at home.

I don't have a My Aged Care assessment yet. Can I still start?

You are always welcome to access our services privately (without government funding) while you wait for your aged care assessment to be completed. Many clients start this way and transition to their funded package once approved. Alternatively, contact My Aged Care on 1800 200 422 to start the assessment process — it's usually quick, and we can continue to help in the meantime.

Referring your clients to Beachside EP

We make the referral process easy for care coordinators and case managers. We accept referrals via phone or email and provide detailed written reports back to you after every review appointment.

What we offer your clients

Accredited Exercise Physiology, Dietetics, and Credentialled Diabetes Education — all from a single allied health practice in Mordialloc. Our clinicians write detailed reports back to the referring care coordinator after every review, keeping you informed and care plans current.

We see clients in-clinic in Mordialloc (Monday–Friday 7am–6pm, Saturday 7am–11am) and via telehealth for those who prefer or require it.

We accept CHSP, Support at Home (SAH), DVA, NDIS, WorkCover, TAC, Medicare CDM, and private clients.
