Referral types accepted
We accept referrals across all major funding streams — no paperwork hassle for your practice.
Medicare — GP Management Plans
Chronic Disease Management plans and Team Care Arrangements including Exercise Physiology. Up to 5 subsidised sessions per calendar year.
DVA — Gold & White Card
DVA-approved provider. Gold and White Card holders accepted. We handle all DVA billing directly — no cost to the patient.
NDIS
Registered NDIS provider. NDIA-managed, plan-managed and self-managed accepted. Functional capacity assessments and progress reports available.
WorkCover Victoria & TAC
WorkCover and TAC approved. Sessions billed directly — no out-of-pocket for the client. Return-to-work programs available.
Private Health
All major private health funds accepted. Clients claim directly through their fund.
Self-referral
No referral required for self-funded clients. They can book directly online or call 0409 051 260.

Clinical, structured and measured — we take care of your patient and keep you informed.
Full clinical assessment
60-minute initial assessment — condition, medications, recent bloods, movement, strength and functional capacity. Written report to you within 1 week.
Structured program
Progressive, measurable exercise program with clear milestones — adjusted based on the patient’s response and your clinical guidance.
Integrated dietetics
For appropriate patients, our Accredited Practising Dietitian works alongside the EP — nutrition and exercise designed as one integrated plan.
Regular reporting
Written progress reports after every review. You stay informed — and your patient’s care team stays aligned.
